Forms for Filing a Bankruptcy Case

A combination of different forms must be used to file a voluntary bankruptcy case, including a Bankruptcy Petition, Schedules, and Statements. Bankruptcy Petition Packages contain the Clerk's Filing Instructions, the forms required for filing a bankruptcy case, and basic instructions.

Local Bankruptcy Forms

Each federal court maintains their own local court forms. Local bankruptcy forms are approved by the Southern District of Florida Bankruptcy Court. 

Official/Director's Bankruptcy Forms

The Official Bankruptcy Forms are approved by the Judicial Conference of the United States under Bankruptcy Rule 9009 to file and take action in bankruptcy cases and adversary proceedings. The Director of the Administrative Office of the United States Courts may issue additional forms for use under the Code. Mortgage Modification Mediation Forms

MMM forms have been approved for use in the Southern District of Florida’s Bankruptcy Mortgage Modification Mediation program as local forms.

Student Loan Program Forms (SLP)

SLP forms have been approved for use in the Southern District of Florida’s Bankruptcy Student Loan Pogram as local forms.

The original text of court-approved forms cannot be modified, with certain exceptions [see LR 9009-1 Local forms and 9004-1(A) Official Forms and Local Forms ].
